Health Care Diary

At Think Pacific we require all health incidents to be well documented for our records and liability. This is becoming increasingly important as we continue to strive towards having the highest standards in the industry. Each year we are working with more global universities and universities have different safety policies for their students that we must comply with. 

Therefore to maintain our integrity as an organisation and positive relationships with partners, we require you – as leaders and first responders, to provide us with this information. 

You have been provided with a healthcare diary template as shown below: 

Key information:

  • Please allocate a diary per volunteer to keep individual records separate.
  • Keep health diaries alongside project details and any further health forms secure and private, we do not want sensitive, confidential health information to be seen by anyone but our staff.
  • You MUST record all health information through daily healthcare diaries for ALL participants’ who are unwell (It is vital we have a written record of any illness on project in case parents, universities or insurance providers require a timeline of illness). 
  • You do not need to communicate this information to PCs immediately but please use them as reference for group health updates for your scheduled check-ins with your PC on Mon, Wed and Friday. 
  • Send photos of health diaries an hour before scheduled chat so PC has time to review information (?)

During your briefing in Suva leaders will be designated different responsibilities which will be split between expenses/health admin/ project diary. Whoever is delegated oversight of health will be responsible for ensuring health diaries are completed to a high standard and checking that updates reach the coordinator. Gold standard – all healthcare diaries completed immediately after a conversation with a volunteer e.g. after a health clinic, or after an ad hoc chat if possible (you may wish to use a template on your phone to record details if mobile). Please note, although one member of the leadership team has oversight of health (quality control/accountability) – ALL leaders should be actively involved with health clinics, writing healthcare diaries, health check ins and support.
